For those needing follow-up care regarding the medical findings from their medical checkup
[Important] Notice for online consultation/telephone consultation
Currently, our clinic offers online & telephone consultations in accordance with the Ministry of Health, Labor and Welfare's orders.*1
Individuals suspected of having coronavirus infection*2are also eligible; therefore please consider taking our remote consultation if you have any fever or other flue like symptoms.

*2 If you would like us to send you an antigen kit, it will cost an additional 3,300 yen, including shipping costs.
In principle, this service is available for return visits, but please call us first for a consultation if you are making a first visit.

| How to make an appointment | You can arrange your appointment through our clinic's website. *Please contact us if you wish to have an online consultation on the same day, or if you are new patient. |
| System charges | Other than the consultation fee, we charge 1,100 yen (tax included) as an online management fee and additionally 275yen for system charge fees. |
| Payment method | Credit cards only *We will send out payment information through SMS to your phone number after the consultation. |
| Prescription | If medications were prescribed, we will FAX the prescription to your preferred pharmacy. *Please make sure to decide your preferred pharmacy in prior to the consultation. |
| Cautions | ・Please be aware that depending on the doctor's consultation, we may ask you to visit our clinic or recommend you to visit another medical institution according to your symptoms. ・If we were to send the antigen kit as a result of your diagnosis, you will be charged an additional 3,300 yen, including the shipping cost. |

Entry of Web Medical Questionnaire
- Once you have confirmed your appointment, please insert a photo of your insurance card, medication booklet information, and fill in the medical questionnaire.

Day of consultation
- Online consultation will be available via Facetime for iPhone.
For Android, we will send you a Google Meet link via email, to access and connect.
If you wish for a telephone consultation, we will contact you. -
*Time may vary depending on the consultation situation on that day. We ask for your kind understanding and cooperation. (Those who booked the morning slot, we will contact you between 10:00 to12:00, afternoon slots will be between 15:00 to 17:00.)
*If you are a new patient, the following restrictions may apply in prescribing the medications due to circumstances such as not knowing the patient's past medical history.
・Maximum number of days of prescriptions is 7 days
・Limitations in prescribing narcotics, psychotropic drugs, and other medications that cannot be safety administered without a face-to-face consultation/examination with a specialist.

Payment/Prescription
- After consultation, a short message via SMS will be sent to the patient's phone number along with a URL attached that leads to further payment information.
Please process the payment immediately. - If you were prescribed with medications, we will fax the prescription to your preferred pharmacy. Therefore,please fill in the pharmacy details through the online medical questionnaire.
Please contact the pharmacy when receiving your medication.
