Nigen Dock Gift Application Process
| 1. Application | Please apply for the gift dock by fax. FAX: 03-5413-1021 Click here for the fax application form (PDF: 34KB) |
| 2. Sending your invoice | Once the health check menu has been decided, we will send you an invoice by mail. |
| 3. Deciding on a consultation date | After confirming your payment, we will decide the date of your medical checkup. If you would like to have your medical checkup on a specific day, such as your birthday, we recommend that you apply early. |
| 4. Pre-test kits etc. will be sent by mail | We will send you a pre-test kit by mail prior to your appointment. |
| 5. Medical Checkup | On the day, please submit the pre-test kit to the reception desk and then your health check will begin. |
| 6. Results Report | You will receive an email notifying you of the results of your online health check approximately three weeks after the test. *Please note that there may be delays depending on the content of the test. |
