The Customer Privacy Policy establishes the Clinic's basic stance in regards to the handling of personal information of persons receiving services (hereafter "Customer").
Specifically, the personal information referred to here is the personal information designated by Article 2, Paragraph 1 of the Act on the Protection of Personal Information.

Article 2 The term "personal information" as used in this Act shall mean information about a living individual which can identify the specific individual by name, date of birth or other description contained in such information (including such information as will allow easy reference to other information and will thereby enable the identification of the specific individual).
In the case of the Clinic collecting personal information from the Customer, collection shall be done within the scope of the healthcare to be received by the customer (e.g. examination, etc.). In the case of using personal information for any other purpose, prior consent as to the purpose and term of use must be obtained.
The following are purposes for with the Clinic may use Customer personal information.
The Clinic will not use the Customer's private information, except in the following cases, outside of the parameters described above. Additionally, except in situations prescribed by law, personal information will not be provided to third parties without the Customer's permission.
In regards to the Customer's personal information, the Clinic will work to maintain correct and up-to-date information, and prevent the leakage, loss, destruction, falsification, and unauthorized access to the Customer's personal information. In the case of contracting the handling of personal information to an external vendor within the limit necessary for the accomplishment of the purpose of use, the Clinic will instruct the contracted party regarding the safe management of personal information.
In the case of the Clinic providing the Customer's personal information to a third party, it shall be done according to any of the use purposes noted in the above Use for providing information out of the Clinic, and data provided through document, film, electronic means, magnetic means, or verbal means, etc. shall be limited to that necessary for the purpose.
In this case, upon request from the Customer himself, submission of personal data that identifies the Customer will be ceased. In the case that submission of personal information to other third parties, personal information will be provided to the third party on receiving consent from the Customer again.
If confirmation or disclosure of personal information is requested by the customer himself or a legally appointed representative, the Clinic shall speedily and properly deal with said request according to law and the Clinic's policies. If the customer himself or a legally appointed representative requests that personal information be amended, added to, deleted, ceased being used, destroyed, or ceased be provided to third parties based on the reason that it is inaccurate, etc. an investigation into the reason shall be commenced speedily and said request shall be dealt with according to law.
