Policy for the protection of personal information for patients receiving services

The Customer Privacy Policy establishes the Clinic's basic stance in regards to the handling of personal information of persons receiving services (hereafter "Customer").
Specifically, the personal information referred to here is the personal information designated by Article 2, Paragraph 1 of the Act on the Protection of Personal Information.

Article 2 The term "personal information" as used in this Act shall mean information about a living individual which can identify the specific individual by name, date of birth or other description contained in such information (including such information as will allow easy reference to other information and will thereby enable the identification of the specific individual).

1. Collection of personal information

In the case of the Clinic collecting personal information from the Customer, collection shall be done within the scope of the healthcare to be received by the customer (e.g. examination, etc.). In the case of using personal information for any other purpose, prior consent as to the purpose and term of use must be obtained.

2. Purpose of use of personal information

The following are purposes for with the Clinic may use Customer personal information.

  • Clinic internal usage
    • For providing healthcare services.
    • For invoicing of medical expenses. For office work regarding healthcare, nursing care, workers accident compensation insurance, and publicly funded healthcare.
    • The Clinic's administrative operations. Creating of basic documentation for the maintenance and improvement of accounting, bookkeeping, internal reporting, healthcare services, etc.; healthcare intern cooperation, hospital case study focused on healthcare improvement and dissemination of information to the public.
  • Use in the contracting of business activities outside the clinic
    • Contracting of laboratory testing, etc.
    • Contracting of healthcare, nursing care, workers accident compensation insurance, and public expense healthcare affairs.
    • Contracting of the maintenance and/or administration of information processing systems.
  • Use for providing information out of the Clinic
    • Partnerships with other internal and external hospitals, clinics, pharmacies, home visiting nursing stations, etc. Graphical information, etc. registration with data centers with that purpose.
    • Answers to requests from other medical institutions, etc.
    • In the case of requesting opinions or advice from an external doctor, etc. for the Customer's examination, etc.
    • Explanation of condition to family, etc.
    • Use for submitting receipts to a screening payment facility, responding to requests from screening payment facilities or insurers, and healthcare expenses invoicing for other healthcare, nursing care, and workers accident compensation, etc.
    • Submitting receipts to government institutions for publicly funded healthcare, responding to requests, and invoicing for other publicly funded healthcare.
    • Notification of results to businesses for health examinations, etc. contracted by said businesses.
    • Consultation and submission of forms to specialized organizations and insurance companies related to medical malpractice insurance.
    • Submission of information to external auditing organizations.

The Clinic will not use the Customer's private information, except in the following cases, outside of the parameters described above. Additionally, except in situations prescribed by law, personal information will not be provided to third parties without the Customer's permission.

  • Notification of results to businesses for health examinations, etc. contracted by said businesses.
  • Consultation and submission of forms to specialized organizations and insurance companies related to medical malpractice insurance.
  • Submission of information to external auditing organizations.

3. Proper management of personal information

In regards to the Customer's personal information, the Clinic will work to maintain correct and up-to-date information, and prevent the leakage, loss, destruction, falsification, and unauthorized access to the Customer's personal information. In the case of contracting the handling of personal information to an external vendor within the limit necessary for the accomplishment of the purpose of use, the Clinic will instruct the contracted party regarding the safe management of personal information.

4. In the case of providing personal information to a third party

In the case of the Clinic providing the Customer's personal information to a third party, it shall be done according to any of the use purposes noted in the above Use for providing information out of the Clinic, and data provided through document, film, electronic means, magnetic means, or verbal means, etc. shall be limited to that necessary for the purpose.
In this case, upon request from the Customer himself, submission of personal data that identifies the Customer will be ceased. In the case that submission of personal information to other third parties, personal information will be provided to the third party on receiving consent from the Customer again.

5. Confirmation and disclosure of personal information

If confirmation or disclosure of personal information is requested by the customer himself or a legally appointed representative, the Clinic shall speedily and properly deal with said request according to law and the Clinic's policies. 6. Amendment, etc. of personal information

If the customer himself or a legally appointed representative requests that personal information be amended, added to, deleted, ceased being used, destroyed, or ceased be provided to third parties based on the reason that it is inaccurate, etc. an investigation into the reason shall be commenced speedily and said request shall be dealt with according to law.
